For your convenience this form allows you to request an appointment for a date 3 days in the future. If you would like to be seen sooner or are having a dental emergency please call the office at (908) 333-4995 for sooner appointment as we always accommodate emergencies the same day and often have same day availability for new and existing patients.
If you have eligible benefits remaining, met your deductible requirements and insurance where a co-pay is not required, your exam, x-rays and cleaning will be at no cost to you. If your insurance requires a co-pay you must pay the required out-of-pocket cost.
